YOU CAN PICK UP A PACKET AT OUR OFFICE ANYTIME, MONDAY-FRIDAY 7:30AM – 4:00PM (CLOSED FOR LUNCH FROM 12:00PM-12:30PM).
***PLEASE BE AWARE THAT PRIOR RESIDENTS OF SUBSIDIZED HOUSING MUST HAVE PAID IN FULL ANY BALANCE OWED TO ANY HOUSING AUTHORITY OR INCOME-BASED PROPERTY TO BE ELIGIBLE FOR OUR WAITING LIST***
Please ensure you have completed all items and instructions below, or we will not be able to complete the application process for you to determine eligibility. The instructions will tell you all information you need to know in order to fill out an application to determine eligibility to be added to the waiting list.
After completing your online application:
1. Come by our office to pick up a packet for all adults in the household.

2. We only accept applications on specific days every month. You can bring your completed paper packets on our designated dates to our Resource Center.

ALL ADULTS ON YOUR APPLICATION WILL HAVE TO BE PRESENT TO VERIFY IDENTITY.
You will also need to bring the below items with your packets.
( ) Social Security cards for all household members
( ) Certified Birth Certificates for all household members
( ) Picture I.D. for all household members age 18+
( ) Income verification/documentation for all household members such as:
a. Most Recent Tax Return
b. Financial Statements (Bank, Chime, Venmo, Cash App, etc.)
c. Social Security or SSI Award Letters
d. The last 4 paystubs from any job, or net income for self-employment
e. Unemployment Payments
f. Child Support Payments and Case Numbers
g. AFDC, SNAP (food stamps), and Families First Benefits
h. Pensions, annuities, retirement accounts, savings accounts, other assets
i. Any other income from any source
